Abschätzung der linksventrikulären systolischen Funktion bei Patienten mit schlechter Bildqualität

Zusammenfassung

Die Bewertung und Quantifizierung der linksventrikulären (LV) systolischen Funktion ist meist die zentrale Fragestellung im Rahmen der klinischen Echokardiographie. Viele Patienten erlauben ein visuelles Abschätzen der systolischen Funktion, allerdings machen eingeschränkte Schallbedingungen alternative Quantifizierungsmethoden notwendig. Dieser Übersichtsartikel stellt die wichtigsten echokardiographischen Methoden vor, die in Bezug auf die Evaluation der systolischen LV-Funktion bei Patienten mit schlechter Bildqualität hilfreich sein können [endokardiale Delineation mittels Kontrastmittel; MAPSE („mitral annular plane systolic excursion“); systolische Mitralringgeschwindigkeit, abgeleitet mittels Gewebedoppler; dP/dtMax, abgeleitet aus dem Profil der Mitralregurgitation; Abschätzung der Pumpfunktion des LV mittels Dopplerechokardiographie]. Es wird jeweils eine kurze Einführung in die Problematik gegeben und eine geeignete Messmethode vorgeschlagen. Ergänzend werden die Indikationen, die klinischen Implikationen und die Grenzen der verschiedenen Methoden diskutiert.
