Additive hormonelle Therapie bei der assistierten Reproduktion

Estradiol, Progesteron und Gonadotropin-Releasing-Hormon-Agonisten

Zusammenfassung

Der vorliegende Beitrag fasst den Kenntnisstand zur Estradiol-, Progesteron- bzw. Gonadotropin-Releasing-Hormon-Agonisten(GnRHa)-Anwendung in der Lutealphase bei der assistierten Reproduktion im März 2014 zusammen. Die Applikation von Estradiol beeinflusst die Therapieergebnisse nicht. Die Progesteronsupplementierung erhöht die Lebendgeburtenrate nach Gonadotropinstimulation signifikant. Eine prolongierte Applikation von Progesteron nach dem positiven Humanen-Choriongonadotropin(HCG)-Test hat keinen weiteren Einfluss auf die Lebendgeburten- und Abortrate. In stimulierten In-vitro-Fertilisation(IVF)- bzw. Intrazytoplasmatische-Spermieninjektion(ICSI)-Zyklen steigert die einmalige Applikation eines niedrig-dosierten GnRHa 5 bzw. 6 Tage nach der Follikelpunktion sowohl im Agonisten- und auch Antagonistenprotokoll die Rate fortlaufender Schwangerschaften nach heutigem Kenntnisstand nicht.
