Aktueller Stellenwert der parametrischen Echokardiographie beim Management von Kardiomyopathien

Früherkennung, Differenzialdiagnose und echokardiographisches Therapiemonitoring

Zusammenfassung

Die parametrische Echokardiographie erlaubt eine Analyse der myokardialen Geschwindigkeiten (z. B. mittels Gewebedoppleranalyse, „tissue Doppler imaging“ [TDI]) und der myokardialen Verformung (Strain, Strainrate, z. B. mittels Speckle-tracking-Echokardiographie [STE]). Dadurch wird eine objektive und quantitative Analyse der kardialen Funktion bei Herzerkrankungen und Kardiomyopathien ermöglicht. Mittlerweile gibt es eine umfangreiche Datenlage, die den Nutzen dieser neuen Methode bei der Früherkennung, der Differenzialdiagnose und dem echokardiographischen Monitoring belegt. Zusätzlich kann eine Gewebecharakterisierung zur Detektion myokardialer Fibrosierungen und Vernarbungen vorgenommen werden.
Heute sollte die parametrische Echokardiographie bei der Beurteilung der diastolischen Funktion (E/E‘-Ratio), der Differenzierung zwischen restriktiver Kardiomyopathie und Perikarditis constrictiva sowie bei der Differenzierung zwischen einer hypertrophen Kardiomyopathie und einem Sportlerherz zur echokardiographischen Standarduntersuchung gehören.
