Anwendungsgebiete der intraoperativen transösophagealen 3D-Echokardiographie

Zusammenfassung

Die intraoperative transösophageale Echokardiographie (TEE) ist heutzutage ein etabliertes diagnostisches Verfahren. Bei Herzklappenoperationen oder komplexen herzchirurgischen Eingriffen gilt die TEE bereits seit geraumer Zeit als Standardverfahren zum intraoperativen Monitoring einerseits und zur chirurgischen Entscheidungsfindung andererseits. In einigen Bereichen wird die intraoperative TEE zudem zur Beurteilung der Hämodynamik und zur Akutdiagnostik eingesetzt. Vor wenigen Jahren wurde die intraoperative 2D-TEE um die Möglichkeit der 3D-Bildgebung in Echtzeit erweitert. Die Überlegenheit der 3D-TEE zeigt sich v. a. durch eine leichtere Zuordnung von pathologischen Veränderungen zu anatomischen Strukturen. Dieser Vorteil ist insbesondere in der Mitralklappenchirurgie von Bedeutung und hat zu einer großen Zahl an Studien geführt, die einen Benefit der 3D-TEE in Echtzeit belegt haben. Auch in der Beurteilung der ventrikulären Funktion hat die 3D-TEE in Echtzeit Vorteile. Eine vollständige 2D-TEE-Untersuchung kann deshalb sehr gut durch eine 3D-Bildgebung ergänzt und verfeinert werden. In der vorliegenden Übersichtsarbeit werden die verfügbaren Daten zur intraoperativen 3D-TEE zusammengefasst und praktische Einsatzgebiete dieser Technik präsentiert.
