Ausrissfraktur des Processus styloideus ulnae bei der distalen Radiusfraktur

Zusammenfassung

Die Fraktur des Processus styloideus ulnae (PSU) tritt in ca. der Hälfte der distalen Radiusfrakturen auf. Ihre Häufigkeit steigt mit der Schwere der distalen Radiusfraktur. Über die Notwendigkeit einer operativen Stabilisierung des PSU herrschte lange Zeit Uneinigkeit. Neuere Studien belegen, dass die Fraktur des PSU ein Prädiktor für ein schlechtes klinisches Ergebnis nach einer distalen Radiusfraktur ist, dass dieses aber unabhängig davon ist, ob die PSU-Fraktur heilt oder eine Pseudarthrose entsteht.
Da die Osteosynthese des PSU mit dem Risiko einer Verletzung des dorsalen Ulnarisastes einhergeht, sollte die Indikation streng gestellt werden. Initial ist diese nur bei einer groben Instabilität des distalen Radioulnargelenks indiziert. Vor einer sekundären PSU-Osteosynthese sollte nach anderen Schmerzursachen, wie einer Schädigung des TFCC, gesucht und diese mit behandelt werden, um eine Schmerzfreiheit zu erzielen.
