Ballonsakroplastie: C-Bogen-kontrolliert oder CT-gesteuert?

Erfahrungen bei 46 Patienten

Zusammenfassung

Einleitung

Sakruminsuffizienzfrakturen werden häufig übersehen und führen zu starken therapierefraktären Schmerzen. Die Fraktur lässt sich am sensitivsten mit der MRT darstellen. Mit der Sakroplastie steht eine Methode zur Verfügung, die Brüche wie bei der Ballonkyphoplastie mit Zement zu fixieren.

Fragestellung

Können Schmerzen mit der Methode gelindert werden und bringt die CT-gesteuerte Zementapplikation weniger Paravasate und Komplikationen als die C-Bogen-kontrollierte Technik?

Material und Methode

In einer retrospektiven Multicenterstudie wurden 46 Patienten (41 Frauen, 5 Männer) mit einem Durchschnittsalter von 75 Jahren von 2 Therapeuten sakroplastiert. Fünfundzwanzig Patienten wurden CT-gesteuert und 21 C-Bogen-kontrolliert behandelt. Die Schmerzen wurden nach der visuellen Analogskala bewertet. Das Follow-up betrug 6 Monate.

Ergebnisse

Die mittlere Versorgungszeit betrug 35 min und die postinterventionelle Krankenhausliegezeit 4 Tage. Die Schmerzen konnten mit der CT-gesteuerten Technik von präoperativ 8,8 + 0,7 auf postoperativ 2,6 + 0,6 (p < 0,001) und mit der C-Bogen-gesteuerten Technik von präoperativ 8,2 + 1,0 auf postoperativ 2,2 + 1,4 (p < 0,001) gesenkt werden. Bei der CT-gesteuerten Technik kam es zu keinen Zementparavasaten (0 von 25 = 0 %), während es bei der C-Bogen-gesteuerten Technik zu 6 asymptomatischen Paravasaten kam (8 von 21 = 38 %). Zusätzlich kam es zu 2 Verletzungen der A. glutaea superior mit Hämatomen im Punktionsgebiet, von denen eines operativ ausgeräumt wurde. Zwei Patienten aus der CT-Gruppe verstarben im Verlauf an Lungenerkrankung und Schlaganfall unabhängig von der Operation.

Schlussfolgerung

Die Ballonsakroplastie führt bei Sakruminsuffizienzfrakturen zuverlässig zur signifikanten Schmerzreduktion. Bei der C-Bogen-kontrollierten Technik sind Zementparavasate und Komplikationen signifikant häufiger als bei der CT-gesteuerten Technik.
