Zusammenfassung

Lungenprotektive Beatmung ist ein Standard in der Therapie des akuten Lungenversagens („acute respiratory distress syndrome“, ARDS) zur Aufrechterhaltung eines suffizienten Gasaustauches bei gleichzeitiger Minimierung beatmungsassoziierter Lungenschäden. Neben der Reduktion der Tidalvolumina ist die Begrenzung der Beatmungsdrücke, die Einstellung eines individuellen positiven endexspiratorischen Drucks und der Erhalt von Spontanatmungsaktivität abzuwägen. Es gibt mehrere bettseitige Techniken, um die Beatmung dem individuellen Patienten und dessen eingeschränktem Gasaustausch anzupassen. Der Beitrag gewährt einen Überblick über die Pathophysiologie des ARDS sowie die beatmungsassoziierten Lungenschädigung und stellt die aktuelle Studienlage zur Einstellung der maschinellen Beatmung beim ARDS dar.
