Behandlung proximaler Humerusfrakturen in Deutschland

Zusammenfassung

Hintergrund

Für dislozierte Humeruskopffrakturen existieren unterschiedliche Therapieverfahren. Untersucht wurde der Einfluss der Krankenhausversorgungsstufe und der Behandlungsfrequenz auf die Versorgungssituation in Deutschland.

Material und Methoden

Es wurden 576 Kliniken eingeschlossen. Die Erhebung umfasste Fragen zur Häufigkeit, Diagnostik, Klassifikation, Therapie, Komplikationen und Fallbeispiele.

Ergebnisse

Den Fragebogen beantworteten 48 % der Kliniken; 73 % behandeln >60 % der Frakturen operativ mit großteils winkelstabilen Implantaten. Für junge Patienten ist die winkelstabile Platte das Implantat der Wahl. Bei älteren Patienten werden auch andere Verfahren angewendet. Probleme und Komplikationen stellen Schwierigkeiten bei der Reposition, eine sekundäre Frakturdislokation, Schrauben-/Bolzenperforation, Humeruskopfnekrose und Impingement dar. Bezogen auf die Indikation, die Implantatwahl und Angaben zu den häufigsten Problemen und Komplikationen wurden keine Unterschiede zwischen den Krankenhäusern unterschiedlicher Versorgungsstufe und der Behandlungsfrequenz deutlich.

Schlussfolgerung

In Deutschland besteht ein Trend zur operativen insbesondere kopferhaltenden, winkelstabilen Osteosynthese mit homogenem Standard unabhängig von der Versorgungsstufe der Einrichtung und der Anzahl der behandelten proximalen Humerusfrakturen pro Jahr.
