Zusammenfassung

Hintergrund

Die Behandlungskonzepte des Rektumkarzinoms haben sich im letzten Jahrzehnt stark verändert und werden kontinuierlich weiterentwickelt. Moderne Rektumchirurgie findet heute im Rahmen multimodaler Therapiekonzepte in darauf spezialisierten High-Volume-Zentren mit guten Kurz- und Langzeitergebnissen statt.

Ziel

Evidenzbasierter Übersichtsartikel zu aktuellen Konzepten in der onkologischen Rektumchirurgie.

Material und Methoden

Mittels systematischer Literaturrecherche in Pubmed, Medline und manueller Recherche wurden relevante Publikationen bis September 2014 untersucht.

Ergebnisse

Die flächendeckende Einführung der totalen mesorektalen Exzision hat maßgeblich zum deutlich verbesserten Langzeitüberleben von Patienten mit einem Rektumkarzinom geführt. Multimodale Therapiekonzepte sind heute ebenso wie laparoskopische Resektionen, Standard für individuell selektionierte Patientengruppen.

Schlussfolgerung

Rektumchirurgie findet heutzutage mit niedriger operativer Morbidität und Mortalität sowie guten onkologischen Langzeitergebnissen an spezialisierten High-Volume-Zentren statt.
