Chronische Schmerzen, Schmerzkrankheit und Zufriedenheit der Betroffenen mit der Schmerzbehandlung in Deutschland

Ergebnisse einer repräsentativen Bevölkerungsstichprobe

Zusammenfassung

Hintergrund

Aus Deutschland liegen nur wenige Daten aus der allgemeinen Bevölkerung zur Prävalenz einer Schmerzkrankheit und der Zufriedenheit von Menschen mit chronischen Schmerzen mit der Schmerztherapie vor.

Methodik

Personen ≥ 14 Jahre einer repräsentativen Stichprobe der deutschen Gesamtbevölkerung wurden im Rahmen einer Querschnittsstudie mit standardisierten Fragebögen im Jahr 2013 untersucht. Chronische Schmerzen wurden basierend auf der Definition der International Association of the Study of Pain und eine Schmerzkrankheit durch das Vorliegen eines beeinträchtigenden Schmerzes im Chronic Pain Grade Questionnaire und vermehrter psychischer Symptombelastung im Gesundheitsfragebogen für Patienten PHQ 4 erfasst. Weiterhin wurde im Fall chronischer Schmerzen nach der Zufriedenheit mit der aktuellen Schmerztherapie und nach einer Behandlung durch einen Schmerztherapeuten gefragt.

Ergebnisse

2508 (57,5 %) der 4360 kontaktierten Personen nahmen an der Studie teil. Die Prävalenz des chronischen, nichttumorbedingten Schmerzes lag bei 26,9 % [95 % Konfidenzintervall (KI), 25,2–28,6 %). Die Kriterien eines chronischen, beeinträchtigenden, nichttumorbedingten Schmerzes erfüllten 7,4 % Personen (95 % KI 5,0–9,9 %), 2,8 % (95 % CI 2,2–3,4 %) der Teilnehmer erfüllten die Kriterien einer Schmerzkrankheit. Eine aktuelle Behandlung ihrer Schmerzen durch einen Schmerztherapeuten gaben 31,9 % der Personen mit beeinträchtigendem Schmerz an. Von diesen waren 33,1 % mit der aktuellen Schmerztherapie (sehr) unzufrieden. Eine Behandlung durch einen Schmerztherapeuten war mit einer höheren Inanspruchnahme anderer ärztlicher Leistungen und einer geringeren Behandlungszufriedenheit assoziiert.

Schlussfolgerungen

Es besteht ein Optimierungsbedarf in der Versorgung von Menschen mit chronischen, beeinträchtigenden Schmerzen. Ob eine spezialisierte Schmerzbehandlung (kosten)effektiv ist, muss durch Längsschnittstudien überprüft werden.
