Das komplexe Beckentrauma

Matching des Beckenregisters DGU mit dem TraumaRegister DGU®

Zusammenfassung

Hintergrund

Komplexe Beckentraumen (KBT), d. h. Beckenfrakturen mit peripelvinen Begleitverletzungen, zeigen bis heute eine hohe Mortalität von etwa 18 %.

Material und Methoden

Retrospektive Auswertung eines Schnittmengendatensatzes aus TraumaRegister DGU® und Beckenregister DGU der Jahre 2004–2009. Verglichen wurden KBT mit nicht-komplexen Beckentraumen (Non-KBT) hinsichtlich Vitalparameter, Notfallmanagement, Intensivaufenthalt und Outcome.

Ergebnisse

Von den 344 Patienten betrug der Anteil an KBT 21 % mit einer im Vergleich zu den Non-KBT signifikant höheren Mortalität (16,7 % vs. 5,9 %). Während präklinisch nur geringe Unterschiede auffielen, so zeigt sich im Schockraum der Anteil an Patienten im Schock und mit traumaassoziierter Gerinnungsstörung bei den KBT signifikant höher, was sich bis zur Aufnahme auf Intensivstation in höheren Infusionsmengen und Transfusionen widerspiegelte. Später auf Intensivstation zeigten die KBT signifikant höhere Raten an Multiorganversagen (MOV) und einen insgesamt längeren Intensivaufenthalt.

Schlussfolgerung

Die Vermeidung eines frühen Verblutungstodes sowie die Therapie intensivmedizinischer Komplikationen stellen auch in Zukunft eine große Herausforderung im Management von KBT dar.
