Diagnostische Molekularpathologie bei lymphatischen und myeloischen Neoplasien

Zusammenfassung

Wesentlich länger als bei den soliden Neoplasien bildet die Molekularpathologie einen integralen Bestandteil der hämatopathologischen Tumordiagnostik. Vorrangiges Ziel ist dabei nicht die Prädiktion einer Medikamentenwirksamkeit wie bei den soliden Tumoren, sondern die Abgrenzung der reaktiven von der klonalen Proliferation sowie die Klassifikation der Erkrankung. Bei den malignen Lymphomen sind die häufigsten zu diesem Zweck erfolgenden molekularen Untersuchungen die des Genrearrangements für Immunglobulinschwerketten bzw. den T-Zell-Rezeptor. In dieser Arbeit sollen jedoch nicht die gut etablierten molekularen Nachweise der Klonalität, sondern neue in den letzten Jahren etablierte diagnostisch relevante Assays beschrieben werden. Dazu gehören Mutationen von MYD88 Kodon 265 für das lymphoplasmozytische Immunozytom, BRAF V600E bei der Haarzellleukämie sowie Stat3, Exon 21 bei indolenten T-Zell-Lymphomen. Bei den Neoplasien der myeloischen Differenzierungsreihe werden bei den myeloproliferativen Neoplasien JAK2 V617F, MPL W515, Calretikulin Exon 9 und die BCR-ABL-Junktion untersucht. Bei den myelodysplastisch/myeloproliferativen Neoplasien sind es SRSF2, SETBP1 und CSF3R, deren Mutationen wichtige differenzialdiagnostische Informationen liefern. Besonders vielfältig sind die Gene, die bei myelodysplastischen Syndromen (MDS) von Mutationen betroffen sind und deren Analyse entscheidende differenzialdiagnostische Anhaltspunkte bei der Abgrenzung reaktiver Dysplasien ergeben. Zu den häufigen Veränderungen beim MDS gehören Mutationen von TET2 und verschiedener Spleißfaktorgene.
