Die proximale Humerusfraktur

Zusammenfassung

Die winkelstabile Plattenosteosynthese, vor 10 Jahren mit großer Euphorie begrüßt, ist durch Berichte über häufige Komplikationen in die Kritik geraten. Die aktuelle Literatur zeigt jedoch, dass eine anatomische Rekonstruktion mit winkelstabilen Platten mit einem besseren Ergebnis verbunden ist. Gleichzeitig finden sich jedoch hohe Komplikationsraten, die sowohl von objektiven Fraktur- und patientenbezogenen Faktoren, aber auch von chirurgenbezogenen Faktoren abhängen. Die Analyse von Risikofaktoren hilft Komplikationen zu vermeiden und richtige therapeutische Entscheidungen zu treffen. Intraoperativ müssen Varusfehlstellung, eine solide mediale Abstützung sowie die korrekte Rekonstruktion der Tubercula gelingen. Gelingt keine anatomische Rekonstruktion mit stabiler medialer Abstützung ist ggf. eine Augmentierung, sonst die primäre Frakturendoprothetik abzugrenzen.
