Zusammenfassung

Problematik

Die distale Humerusfraktur stellt aufgrund der komplexen Gelenkanatomie und häufig kleinen Gelenkfragmente an Operateur sowie Implantat besondere Herausforderungen. Auch neigt das Ellenbogengelenk zur Entwicklung einer postoperativen Ellenbogensteife.

Therapie

In der operativen Versorgung setzten sich die winkelstabilen, anatomisch angepassten, orthogonalen oder parallel positionierten Platten unter Stabilisierung des radialen und des ulnaren Pfeilers durch. Als Standardzugang bietet der Paratrizepszugang mit Olekranonosteotomie die beste Gelenkübersicht. Für die Implantation von Endoprothesen gilt der Trizeps-Reflecting-Zugang nach Morrey als besonders muskelschonend. Mit Einführung der winkelstabilen Platten reduzierte sich der Bedarf einer primären Knochentransplantation stark. Bei schwacher Evidenz wird die Transposition des N. ulnaris allgemein empfohlen, wenn dieser Kontakt zum Implantat hat. Die Prothesenversorgung ist bei hochgradiger Osteoporose, bei älteren Patienten und dem Vorliegen kleiner distaler Abscherfragmente indiziert. Hinsichtlich der Ossifikationsprophylaxe besteht die Empfehlung zur Bestrahlung oder der Gabe von NSAR (nichtsteroidale Antirheumatika) bei Patienten mit besonderem Risiko (z. B. schweres Schädel-Hirn-Trauma, rezidivierende Ellenbogenluxationen, verspätete Versorgung usw.).
