Effektivität antibiotischer Initialtherapie in der Behandlung der kontaktlinsenassoziierten bakteriellen Keratitis

Zusammenfassung

Hintergrund

Die kontaktlinsenassoziierte bakterielle Keratitis ist eine Ursache potenziell visusbedrohender kornealer Trübungen. Eine effektive Initialtherapie ist Voraussetzung zur Minimierung von Langzeitkomplikationen. Aufgrund dynamischer Keimspektren dient diese Untersuchung der Effektivitätsprüfung aktuell üblicher empirischer Antibiotikaregime.

Methoden/Patienten

Alle konsekutiven Fälle kontaktlinsenassoziierter Keratitis in der Ambulanz der Universitäts-Augenklinik Innsbruck zwischen Januar 2010 und April 2012 wurden retrospektiv analysiert.

Ergebnisse

In 69 von 123 inkludierten Fällen wurden Keime gezüchtet. In 59,4 % der Fälle wurden grampositive, in 50,7 % gramnegative Bakterienstämme und in 7,2 % Pilzstämme identifiziert. 29,0 % der Fälle waren Mischinfekte. Die Initialtherapie bestand in 87,8 % der Fälle aus einem Aminoglykosid und einem Chinolon der zweiten Generation. Diese war in vitro einer Kombination aus Moxifloxacin und Ciprofloxacin oder Moxifloxacin und Refobacin unterlegen.

Zusammenfassung

Bakterielle Erreger der kontaktlinsenassoziierten Keratitis bleiben durch empirische Kombinationsregime sehr gut behandelbar. Fluorochinolone sind zur Monotherapie nicht geeignet.
