Elektive und akute Eingriffe in der Unfallchirurgie

Revisionspflichtige Komplikationen

Zusammenfassung

Hintergrund

Die Behandlungsverläufe aller operativ versorgten Patienten im Zeitraum 01/2010 bis 03/2012 (n = 5,248) in einer unfallchirurgischen Klinik einer universitären Ausbildungsklinik wurden hinsichtlich des Auftretens von revisionspflichtigen Komplikationen analysiert.

Patienten und Methode

Zwei Patientengruppen (Gruppe 1: akut traumatologische Eingriffe, n = 3942, Gruppe 2: elektive Eingriffe, n = 1306) dienten als Studienkohorten. Bei allen 5248 Patienten wurden die revisionspflichtigen Komplikationen nach Abschluss der stationären Behandlung mittels eines Komplikationsmeldebogens erfasst und quartalsweise in einer eigenen Klinikkonferenz von allen ärztlichen Mitarbeitern der Klinik analysiert. Anschließend erfolgte die Einteilung der Komplikationen in folgende Gruppen: 1) technische Komplikationen, 2) lokale Komplikationen und 3) Infektionen.

Ergebnisse

Bei 4,4 % (n = 233) aller operativ versorgten Patienten war der Behandlungsverlauf durch eine revisionspflichtige Komplikation protrahiert. Mit 2,3 % (n = 123) waren technische Komplikationen die häufigste Ursache für den Revisionseingriff, gefolgt von lokalen Komplikationen (1,3 %) wie postoperativen Hämatomen (0,6 %) oder Wundheilungsstörungen (0,6 %). In der Gruppe der elektiven Eingriffe war der Anteil an revisionspflichtigen Komplikationen mit 3,1 % signifikant niedriger als in der Gruppe akut traumatologische Eingriffe mit 4,9 % (p < 0,05). Im Vergleich zu den Patienten ohne revisionspflichtige Komplikation war das durchschnittliche Patientenalter (54,3 vs. 63,4 Jahre), der Anteil an Patienten mit Nikotinabusus (9 % vs. 18,5 %) und die Dauer des stationären Aufenthalts um 6,7 Tage signifikant (p < 0,05) erhöht. Die Anzahl an Begleiterkrankungen war mit 2,8 vs. 3,8 tendenziell erhöht.

Schlussfolgerung

Die Erfassung und Analyse der revisionspflichtigen Komplikationen ist eine entscheidende Qualitätskontrolle in der Chirurgie. Nicht beeinflussbare Risikofaktoren für Komplikationen sind ein höheres Patientenalter und eine erhöhte Komorbidität. Akuteingriffe sind mit einem höheren Komplikationsrisiko behaftet als elektive Eingriffe, bei welchen eine präoperative Risikoabwägung möglich ist. Vermeidbare technische Komplikationen müssen strukturiert und systematisch analysiert werden, da an dieser Stelle das größte Potential besteht, die Rate an revisionspflichtigen Komplikationen zu senken.
