Zusammenfassung

Technische Innovationen im Bereich der Endoskopie des oberen Harntakts wie optimierte Optiken und geringere Größe flexibler Ureterorenoskope, der Einsatz endoskopischer Laserverfahren und perkutaner Resektionsverfahren haben die Möglichkeiten einer organerhaltenden Therapie bei Urotheltumoren des oberen Harntrakts verbessert. Zudem stehen zur differenzierten Beurteilung von Gewebestrukturen neue endoskopische Filterverfahren zur Verfügung. Während früher die radikale Nephroureterektomie als alleiniger Goldstandard zur Therapie von Tumoren des oberen Harntrakts galt, bleibt ein endoskopisches Vorgehen aktuell somit nicht ausschließlich imperativen Indikationen für einen Nierenerhalt vorbehalten. Die onkologische Effektivität sowohl einer rein endoskopischen Therapie als auch einer offenen segmentalen Ureterresektion ist klinisch belegt. Hierbei ist jedoch der sinnvollen präoperativen Risikobewertung und einer stringenten postoperativen endourologischen Nachsorge höchste Beachtung beizumessen.
