Erfolgreiche Endodontie im Praxisalltag

Zusammenfassung

Die orthograde endodontische Behandlung beinhaltet folgende Etappen: (a) Reinigung und Desinfektion, (b) Aufbereitung und Formgebung sowie (c) dreidimensional stabile und bakteriendichte Füllung des gesamten Wurzelkanalsystems. Die einzelnen Etappen der endodontischen Behandlung zählen zu den aufwendigsten Maßnahmen in der täglichen zahnärztlichen Sprechstunde. Reinigung und Desinfektion des Wurzelkanalsystems erfolgen mechanisch mithilfe manueller oder maschinell betriebener Instrumente in rotierender und neuerdings reziproker Arbeitsweise. Dadurch wird die adäquate Formgebung des Wurzelkanals für die optimale Spüllösungswirkung und anschließende Füllung erreicht. Die chemische Desinfektion des Wurzelkanalsystems erfolgt ultraschallaktiviert mithilfe geeigneter Spüllösungen, wie zum Beispiel Natriumhypochlorit (NaOCl), Chlorhexidin (CHX), Zitronensäure oder Äthylendiamintetraessigsäure (EDTA). Diese Agenzien mit unterschiedlichem Wirkungsspektrum können Wurzelkanalbereiche erreichen, die instrumentell nicht erschließbar sind, und somit zur Eliminierung des bakteriell infizierten Wurzelkanalinhalts beitragen. Verschiedene Studien belegen, dass durch den Einsatz optimierter Behandlungstechniken und entsprechender Instrumente, gepaart mit einem umfassenden Know-how des Behandlers, die Erfolgsquote der endodontischen Maßnahmen sehr hohe Werte erreichen kann.
