Ethische Aspekte der Ernährungstherapie bei Kindern mit Demenzerkrankungen

Zusammenfassung

Kinder mit zu Demenz führenden Hirnprozessen werden oft fehlernährt, was zu vermeidbarer Morbidität führt. In späteren Stadien verlieren sie die Fähigkeit, zu essen und zu trinken. Dann ist zu entscheiden, ob eine Ernährungstherapie das Wohlbefinden steigern kann oder nur ein Leben mit geringer Qualität verlängert. Bei einem nichtmitteilungsfähigen Kind muss sein eigenes bestes Interesse vermutet werden. Dies kann nur durch die Eltern oder einen geeigneten Vertreter geschehen. Die Herstellung der notwendigen Kompetenz der Eltern für vitale Entscheidungen bei ihrem Kind ist ein diffiziler und lang dauernder Prozess in der Verantwortung eines persönlich involvierten Arztes. Wir beschreiben notwendige medizinische und andere Überlegungen mit dem Ziel, professionellen Betreuern praktisch brauchbare Hinweise zu geben. Die individuelle Haltung von Familien gegenüber Entscheidungsmöglichkeiten ist sehr unterschiedlich und nach Möglichkeit zu respektieren. Wir versuchen zu zeigen, wie bei einem Kind mit schwerem Hirnabbau die Entscheidung zum Beginn oder zur Beendigung einer künstlichen Ernährung oder zum völligen Verzicht darauf in einer Weise getroffen werden kann, die für die Beteiligten rational und emotional akzeptierbar ist. Einige der Überlegungen gelten auch für andere Hirnschäden bei Kindern.
