Freigabe von Bewegungssegmenten nach dorsaler Stabilisierung

Auswirkung auf die betroffenen Bandscheiben

Zusammenfassung

Hintergrund

Es bestehen Unklarheiten über den Effekt der Freigabe eines Bewegungssegments auf die Bandscheibe.

Ziel

Ziel dieser Arbeit ist es, radiologische Veränderungen auf die Bandscheibenmorphologie und -funktion nach Materialentfernung (ME) des dorsalen Fixateurs herauszuarbeiten.

Material und Methoden

Bei 19 Patienten wurde nach knöcherner Konsolidierung einer thorakolumbalen Wirbelkörperfraktur (Durchschnitt 13 [8 bis 24] Monate) der dorsale Fixateur entfernt. Bei 10 Patienten erfolgte eine additive ventrale monosegmentale Spondylodese. Durch die ME wurden 28 Bewegungssegmente freigegeben. Bei 10 davon war eine Frakturbeteiligung nachweisbar (verletzte Bandscheiben), bei 18 zeigte sich keine Frakturbeteiligung (gesunde Bandscheiben). Als Referenzbandscheiben wurden die Bandscheibenfächer kranial und kaudal der Instrumentierung erfasst. Vor und nach ME sowie nach 6 Monaten erfolgte eine Zielaufnahme mit Ermittlung der Bandscheibenfachhöhen. Daneben wurden die Bandscheibenfachwinkel in Flexion und Extension erfasst. Die Bandscheibenfunktion wurde aus der Differenz beider Messungen definiert. Zwei unabhängige Untersucher führten die Messungen durch. Die statistische Auswertung wurde mit dem Mann-Whitney-U-Test (Signifikanzniveau: p < 0,05) und „interclass correlation coefficient“ (ICC) durchgeführt.

Ergebnisse und Diskussion

Zwischen Referenz- und überbrückten Bandscheiben ließen sich keine signifikanten Unterschiede bzgl. Bandscheibenfunktion und -fachhöhe nachweisen. Dagegen zeigten sich bei den verletzten Bandscheiben geringere Bandscheibenfachhöhen (p = 0,028) und reduzierte Bandscheibenfachwinkel (p = 0,027) im Vergleich zu den Referenzbandscheiben. Daneben wurden im Falle einer ME innerhalb von 12 Monaten signifikant geringere Bandscheibenfachhöhen in den überbrückten Bandscheiben nachgewiesen, als nach ME während des 2. postoperativen Jahres (p = 0,018).
Die Inter- und Intraobserverübereinstimmung der Bandscheibenfunktion war gut bis sehr gut (Interobserver 0,80; 95-%-Konfidenzintervall [KI] 0,68–0,88; Intraobserver 0,85; 95-%-KI 0,76–0,90), die der Bandscheibenfachhöhe war sehr gut (Interobserver 0,85; 95-%-KI 0,76–0,91; Intraobserver 0,93; 95-%-KI 0,88–0,95).

Schlussfolgerung

Die temporäre Fixierung einer Bandscheibe nach dorsaler Stabilisierung und anschließender Freigabe führt röntgenologisch zu keinem relevanten Höhen- und Funktionsverlust des Bandscheibenfachs. Lediglich bei initialer Frakturbeteiligung des Bandscheibenfachs kommt es zu einer signifikanten Höhenreduktion und zu einem verminderten Extensionspotenzial. Kein positiver Effekt war durch eine Reduktion der Überbrückungsdauer auf weniger als 12 Monate nachzuweisen.
