Hyponatriämie und Tolvaptan

Wo stehen wir 5 Jahre nach der Zulassung?

Zusammenfassung

Das Aquaretikum Tolvaptan ist in Europa seit mehr als 5 Jahren für die Indikation der euvolämen Hyponatriämie infolge eines Syndroms der inadäquaten Sekretion von antidiuretischem Hormon (SIADH) zugelassen. Viele Patienten wurden in den letzten Jahren mit Tolvaptan behandelt und zahlreiche Ärzte konnten praktische Erfahrungen sammeln. Länder, wie die USA, verfügen schon über längere Erfahrungen, z. T. auch mit der Indikation der hypervolämen Hyponatriämie. Nach der Zulassung wurden weltweit mehr als 5000 Patienten in das sog. Hyponatriämie-Register eingeschlossen. Auch aus Deutschland nahmen 22 aktive Zentren mit insgesamt 317 Patienten teil. Während zu diesem abgeschlossenen Register mittlerweile Veröffentlichungen vorliegen, steht die abschließende Publikation der multinationalen „post-authorization safety study“ zu Tolvaptan in der Behandlung des SIADH noch aus. Zu Tolvaptan sind in den Jahren 2012 und 2013 zwei Rote-Hand-Briefe erschienen. Der erste Brief warnt vor dem Risiko zu schneller Serumnatriumanstiege unter Tolvaptan und informiert detailliert, wie das Risiko für eine osmotische Demyelinisation minimiert werden kann. Bislang ist ein gesicherter Fall eines osmotischen Demyelinisationssyndroms (ODS) bei allerdings unsachgemäßem Gebrauch von Tolvaptan in Monotherapie bekannt. Der zweite Rote-Hand-Brief informiert über das potenzielle Risiko einer (reversiblen) Leberschädigung durch Tolvaptan, das sich aus der TEMPO-3:4-Studie ergibt. Hier wurde Tolvaptan in hoher Dosis zur Therapie der autosomal-dominanten polyzystischen Nierenerkrankung eingesetzt. Während die European-Renal-Best-Practice(ERBP)-Leitlinie von 2014 keine Empfehlung für Tolvaptan in der Indikation SIADH ausspricht, kommen andere Leitlinien zu anderen Ergebnissen. Abschließend gibt es auch 5 Jahre nach Zulassung von Tolvaptan keine einheitliche Beurteilung. Zum jetzigen Zeitpunkt sind noch viele Fragen offen. Die Einleitung einer Therapie mit Tolvaptan bleibt dem erfahrenen Arzt im Krankenhaus vorbehalten. Er muss die Behandlung auf Basis seiner klinischen Einschätzung an die individuelle Situation des Patienten anpassen.
