Zusammenfassung

Die kardiale Computertomographie und interventionelle Elektrophysiologie haben sich in den vergangenen Jahren wesentlich weiterentwickelt. Die Computertomographie weist besonders im Hinblick auf die Strahlenreduktion, aber auch durch die Erweiterung der diagnostischen Möglichkeiten Innovationen auf. In vielen elektrophysiologischen Zentren spielt sie eine wichtige Rolle für die Planung und Durchführung interventioneller Prozeduren. Zudem hat sie eine zentrale Bedeutung in der Erfassung relevanter Komplikationen wie einer Pulmonalvenenstenose oder atrioösophagealen Fistel. Besonders primär anatomisch orientierte Ablationsverfahren wie die Vorhofflimmerablation erfordern eine genaue Kenntnis der relevanten kardialen Strukturen. Gerade die ballonbasierten Verfahren können wichtige Detailinformationen aus der präinterventionellen Bildgebung ziehen. Hier ist zu erwarten, dass diese Verfahren mithilfe der exakten anatomischen Information sowohl im Hinblick auf Prozedur- und Durchleuchtungszeiten, aber auch in Bezug auf die Effektivität und Patientensicherheit weiter verbessert werden können. Die Integration anatomischer Bilddaten in dreidimensionale Navigationssysteme ist heute ein fester Bestandteil elektrophysiologischer Untersuchungen. Eine Reduktion der Fluoroskopiezeit, aber auch eine geringere Rezidivrate von Vorhofflimmern nach Pulmonalvenenisolation konnte hierdurch in nicht randomiserten Studien bereits nachgewiesen werden. Die vorliegende Übersicht fasst aktuelle Entwicklungen im Einsatz der kardialen Computertomographie in der interventionellen Elektrophysiologie mit besonderem Fokus auf die Vorhofflimmerablation zusammen.
