Kasuistik
Einer 80jährigen Patientin war seit 3 Monaten eine schmerzlose „Schwellung” im Bereich des rechten medialen Lidwinkels aufgefallen. Seit 2 Wochen sei der Tumor vermehrt gerötet gewesen, und es habe wiederholt daraus geblutet. Ophthalmologisch bestand weiterhin ein weit fortgeschrittenes primäres Offenwinkelglaukom sowie eine Cataracta corticalis provecta. Der VR betrug 0,2, der VL 0,5. Ein Diabetes mellitus war seit 15 Jahren bekannt. Klinisch vermutete man bei dem Tumor ein superinfiziertes Keratoakanthom, differentialdiagnostisch ein exulzeriertes Basaliom. Feingeweblich wurde die Diagnose eines „Epithelioma calcificans Malherbe” (Pilomatrixom) mit zentraler Ulzeration gestellt. Nach histologisch kontrollierter Tumorexzision „im Gesunden” und Rekonstruktion durch ein freies Transplantat vom ipsila-teralen Oberlid ist die Patientin heute - 8 Jahre danach -rezidivfrei.
Summary
An 80-year-old lady noticed a painless “swelling” in the medial canthal region 3 months. The tumor showed a reddish color due to a recurrent superficial bleeding for 2 weeks. Clinically the tumor was supposed to be a super-infected keratoacanthoma. The differential diagnosis included an ulcerative basal cell carcinoma. Excisional biopsy and reconstruction using a free graft from the ipsila-teral upper lid were performed. Ffistopathologically the lesion proved to be a “benign calcifying epithelioma Mal-herbe” (pilomatrixoma). Today, 8 years after surgery, there was no recurrence of the tumor. A circumscribed, subepidermal, movable reddish or livid tumor of the upper lid, eye brow or of the canthal region should be suggestive of pilomatrixoma, not only in young females.
