Zusammenfassung

In dieser Übersichtsarbeit werden die klinischen Präsentationen, die bildgebenden Befunde sowie das diagnostische und therapeutische Vorgehen bei verschiedenen Liquorunterdruckformen dargestellt. Das Liquorunterdrucksyndrom entsteht spontan, nach Lumbalpunktion, Trauma mit Liquorfistel, ungewollter Duraeröffnung oder übermäßiger Liquordrainage bei oder nach intrakraniellen oder spinalen Operationen sowie nach Hydrozephalusshunts mit überdrainierenden Ventilen. Das Leitsymptom – orthostatischer Kopfschmerz – kann selten durch atypische Befunde wie u. a. Koma, frontotemporale Demenz, leptomeningeale hämosideroseassoziierte Symptome maskiert werden. Kranielle und spinale MRT-Befunde sind nicht immer eindeutig, das diagnostische Vorgehen und die Suche nach einem „aktiven“ Liquorleck (keine Suche, CT-Myelographie, digitale Subtraktionsmyelographie, MR-Myelographie mit Gadolinium) werden kontrovers diskutiert. Ebenso ist das therapeutische Vorgehen (konservativ, blinder, Durchleuchtungs- oder CT-gesteuerter Blutpatch, CT-gesteuerte Fibrininjektion, Operation) uneinheitlich.
