Mehrdimensionale Bewegungsanalyse des Handgelenks nach distaler Radiusfraktur

Zusammenfassung

Distale Radiusfrakturen können Bewegungseinschränkungen des Handgelenks verursachen. Das Ziel dieser Arbeit ist es, diese Veränderungen durch eine Bewegungsanalyse des Handgelenks bei Patienten nach Ausheilung der Frakturen zu untersuchen.
Eine multidirektionale Bewegungsanalyse der Handgelenke wurde mittels Elektrogoniometrie bei 28 Patienten nach einseitiger, intraartikulärer distaler Radiusfraktur durchgeführt. Die Patienten wiesen geringe subjektive Restbeschwerden auf (DASH-Score 16,7 Punkte). Für das Experiment wurden die Patienten aufgefordert, mehrfache maximale Zirkumduktionsbewegungen der Handgelenke auszuführen. Anschließend wurde durch ein speziell erzeugtes MatLab-Programm eine Zirkumduktionshülle aus den Winkeldaten rekonstruiert. Die maximal erzeugten Werte in Radial-Ulnar-Abduktion und Extension-Flexion wurden ermittelt und die Gesamtbeweglichkeit als Fläche innerhalb der Zirkumduktionshülle berechnet. Unterschiede zwischen der gesunden und erkrankten Seite wurden mittels Studentʼs t-Test erfasst.
Die Zirkumduktionshüllen der Handgelenke wiesen eine bohnenförmige Konfiguration auf mit einer geneigten Hauptachse, die von Radialabduktion/Extension nach Ulnarabduktion/Flexion verlief. Im Vergleich zur gesunden Gegenseite zeigten sich an den erkrankten Handgelenken eine signifikante Bewegungseinschränkung für die orthogonal zueinander liegenden Hauptbewegungsrichtungen (p < 0.01) und die Flächen innerhalb der Zirkumduktionshüllen (p < 0.01).
Die Ergebnisse demonstrieren, dass nach verheilten distalen Radiusfrakturen noch wesentliche Bewegungseinschränkungen der betroffenen Handgelenke verbleiben können, die unter Umständen der konventionellen Bewegungsmessung durch manuelle Goniometrie verborgen bleiben.
