Mittelfristige Behandlungsergebnisse von 19 Processus-coronoideus-Frakturen und ihren Begleitverletzungen

Zusammenfassung

Hintergrund

Processus-coronoideus-Frakturen sind selten und therapeutisch anspruchsvoll. Mit der retrospektiven Nachuntersuchung der Patienten an einem überregionalen Traumazentrum sollten mittelfristige Behandlungsergebnisse gewonnen und prognostisch relevante Faktoren isoliert werden.

Material und Methoden

Die vollständigen Behandlungsdaten von 26 Patienten aus 5 Jahren (2004–2009) konnten ausgewertet werden. Durchschnittlich 34 Monate (SD 20) nach Unfall konnten 18 Patienten (69 %) klinisch und z. T. nativradiologisch nachuntersucht werden. Die Ergebnisse wurden statistisch ausgewertet.

Ergebnisse

Das Durchschnittsalter betrug 56 Jahre (SD 14), 14 weiblich, 12 männlich, jeweils 13 links bzw. rechts. Frakturtypen am Processus coronoideus ergaben sich nach Regan und Morrey Typ I (10), II (5) und III (11). Als zusätzliche Verletzungen wurden 19 stattgehabte Ellenbogengelenkluxationen, 18 Radiusköpfchenfrakturen, 5 proximale Ulnaschaftfrakturen und 2 Olekranonfrakturen nachgewiesen. Eine durchschnittliche Beweglichkeit von 109° (SD 17°) wurde in Extension/Flexion und eine von 154° (SD 37°) in Pronation/Supination erreicht. Elf von 19 (58 %) Patienten gaben ein stabiles Gelenk an. Der MEPS beträgt durchschnittlich 78 (SD 16; 50–100) Punkte.

Schlussfolgerung

Eine operative Therapie der ligamentären Verletzungen nach Luxationsfraktur des Ellenbogengelenks ist statistisch bisher nicht zu begründen. Eine stabile knöcherne Rekonstruktion des Processus coronoideus ist prognostisch relevant. Eine komplette Radiusköpfchenresektion erhöht die Gelenkinstabilität und ist mit einer signifikant höheren Rate an Fixateurbehandlungen assoziiert.
