Molekulares sIgE Profil bei Säuglingen und Kleinkindern mit Erdnusssensibilisierung und Ekzem

Zusammenfassung

Hintergrund:

Viele Kinder entwickeln bereits im Säuglingsalter eine Sensibilisierung gegen Erdnuss, noch vor dessen Einführung mit der Beikost. Besonders häufig ist eine Sensibilisierung bei Kindern mit Ekzem. Bisher gibt es wenige Daten bezüglich des Erkennungsmusters spezifischer Erdnussallergene für diese Patientengruppe. Ziel der Arbeit war es, ein Allergenprofil von Säuglingen und Kleinkindern mit einer Sensibilisierung gegen Erdnuss und Ekzem zu erstellen.

Methoden:

Seren von 53 Kindern ≤ 20 Monaten mit Ekzem und Sensibilisierung gegen Erdnuss, die aber noch keine erdnusshaltigen Produkte gegessen hatten, wurden in die Analyse einbezogen. Die Seren wurden mittels Microarray Immunoassay (ImmunoCAP ISAC) analysiert.

Ergebnisse:

Insgesamt zeigten 63 % der erdnusssensibilisierten Kinder spezifisches Immunglobulin E (sIgE) gegen mindestens eines der Erdnussallergene im Microarray. Von 40 % der Kinder wurde das 7S-Globulin Ara h 1 erkannt, von 30 % das 2S-Albumin Ara h 2 und von 23 % das 11S-Globulin Ara h 3. Das Bet-v-1-Homolog Ara h 8 wurde von einem Kind erkannt. Bei 24 von 53 Kindern lagen Daten zur klinischen Relevanz vor: 14 von 24 Patienten hatten eine klinisch relevante Erdnussallergie, während zehn Kinder klinisch tolerant waren. Von 43 % (6 von 14) der erdnussallergischen Kinder wurde im Microarray nicht das Samenspeicherprotein Ara h 2 erkannt. Zwei ­dieser sechs Kinder waren monosensibilisiert gegen Ara h 1, zwei gegen Ara h 3 und von drei Kindern wurde keines dieser Samenspeicherproteine erkannt.

Diskussion:

Es konnte gezeigt werden, dass von Säuglingen und Kleinkindern mit Ekzem und einer Sensibilisierung gegen Erdnuss – noch vor deren Einführung in die Beikost – vorrangig Samenspeicherproteine der Erdnuss erkannt werden. Eine Sensibilisierung gegen pollenassoziierte Nahrungsmittelallergene scheint noch keine Rolle zu spielen. Von klinischer Relevanz scheint in diesem Alter nicht nur Ara h 2, sondern insbesondere auch Ara h 1 zu sein.
