„Sudden gains“ und „sudden losses“

Zusammenfassung

Hintergrund

Psychotherapieverläufe sind nicht immer linear. Sudden gains und Sudden losses stellen reliable, bedeutsame und relativ stabile Veränderungen von einer Sitzung zur nächsten dar. Das Auftreten von Sudden gains ist ein positiver Prädiktor für den Behandlungserfolg. Sudden losses hingegen sind mit mäßigem Behandlungserfolg assoziiert.

Ziel der Arbeit

In diesem Beitrag wird der Forschungsstand zu Sudden gains und Sudden losses übersichtsartig dargestellt und exemplarisch um eigene Forschungsergebnisse zu Interaktionsproblemen (Allianzbrüche) in Gain- und Loss-Sitzungen ergänzt.

Material und Methode

Mithilfe kontinuierlicher psychometrischer Verlaufsmessungen wurden Sudden gains und Sudden losses identifiziert. An einer universitären Forschungsambulanz wurden Videoaufzeichnungen von Therapiesitzungen (N = 82 Videos, N = 53 Patienten) auf Brüche in der therapeutischen Allianz und Auflösungsbemühungen der Therapeuten an einer gemischten Stichprobe von Patienten mit hauptsächlich Angst- und depressiven Störungen untersucht.

Ergebnis

Rückzugsbrüche treten ungeachtet des Sitzungstyps (Gain-, Loss- und neutrale Sitzungen) in mehr als 60 % aller Sitzungen auf. Offene Konfrontationsbrüche hingegen sind in Sudden-gain-Sitzungen kaum festzustellen (12 %), jedoch in 42 % aller Sudden-loss-Sitzungen. Zudem reagieren Therapeuten in Sudden-gain-Sitzungen signifikant anders mit bestimmten Strategien auf Beziehungsbrüche, als es Therapeuten in Sudden-loss-Sitzungen tun.

Schlussfolgerung

Die Ergebnisse liefern Ansätze für die Supervision von Therapeuten in schwierig verlaufenden Therapien sowie für das gezielte Feedback über Therapieverläufe mit Handlungsempfehlungen für Therapeuten.
