Wann ist sie behandlungsbedürftig?

Zusammenfassung

Hintergrund

Die osteochondrale Läsion (OCL) ist eine häufig diagnostizierte Läsion und beeinflusst die Lebensqualität des Patienten. Zudem kann die Läsion, bedingt durch die fehlende Kongruenz des Gelenkknorpels in fortgeschrittenen Stadien sowie die fehlenden Regenerationsfähigkeit des hyalinen Knorpels, zu einer frühzeitigen Arthrose führen. Daher besteht häufig die Notwendigkeit einer operativen Therapie. Konservative Behandlungsoptionen sind lediglich in frühen Stadien der OCL erfolgversprechend.

Therapie

Zu den operativen Therapieoptionen zählen neben den stammzellrekrutierenden Verfahren die Knochen-Knorpel-Transplantation sowie die autologe Chrondrozytentransplantation. Die Knochen-Knorpel-Transplantation ist das einzige Verfahren, das den Defekt mit hyalinem, vitalem Knorpel deckt. Hierbei ist die Entnahmemorbidität zu beachten, ebenso der Body-Mass-Index des Patienten. Stammzellrekrutierende Verfahren wie auch die autologe Chondrozytentransplantation führen häufig zu faserknorpeligem, funktionell minderwertigem Gewebe.
