Postoperative implantatassoziierte Osteitis am Schultergürtel

Materialerhaltendes Revisionskonzept mit Einlage einer Dauerdrainage

Zusammenfassung

Hintergrund

Die posttraumatische und postoperative implantatassoziierte Osteitis (PIO) ist eine zunehmend an Bedeutung gewinnende Subgruppe der Knocheninfektion. Insbesondere am Schultergürtel gibt es bisher kein schlüssiges Behandlungskonzept. Ziel dieser Studie war es, ein Therapiekonzept mit einmaligem Débridement und Dauerdrainageneinlage unter vorübergehendem Erhalt des Implantats bei akuter bzw. subakuter PIO am Schultergürtel zu evaluieren.

Patienten und Methoden

Es wurden 31 Patienten mit PIO am Schultergürtel in diese Studie eingeschlossen (14 proximaler Humerus, 10 Klavikula, 7 AC-Gelenk). 27 Patienten konnten mindestens 1 Jahr postoperativ nachuntersucht werden.

Ergebnisse

Der temporäre Materialerhalt bis zur knöchernen und ligamentären Ausheilung gelang bei > 83 % der Patienten. Bei 6 Patienten trat einmalig ein revisionsbedürftiges Infektrezidiv auf. Das klinische Outcome war 1 Jahr postoperativ gemessen am Constant-Score exzellent (91,6 ± 12,8 Punkte).

Schlussfolgerung

Die vorliegende Arbeit beschreibt ein kostengünstiges, einfaches und erfolgreiches Revisionskonzept mit lokalem Débridement und Einlage einer Dauerdrainage zur Behandlung einer PIO am Schultergürtel.
