Präklinische Atemwegssicherung bei Traumapatienten

Erfahrungen mit dem C-MAC®-Videolaryngoskop

Zusammenfassung

Hintergrund

Die Atemwegssicherung hat höchste Priorität in der Traumaversorgung. Voraussetzung zur erfolgreichen endotrachealen Intubation (ETI) ist eine gute laryngoskopische Visualisierung der Stimmbandebene. Ziel dieser Untersuchung ist es, die Erfahrungen mit dem routinemäßigen Einsatz eines Videolaryngoskops bei der präklinischen Intubation von Traumapatienten an einem Luftrettungszentrum zusammenzufassen.

Methodik

Das C-MAC®-Videolaryngoskop basiert auf einem Macintosh-Spatel und wird am Rettungshubschrauber (RTH) „Christoph 22“ als Standardlaryngoskop für alle präklinischen ETI eingesetzt. Über 17 Monate wurden alle präklinischen ETI von Traumapatienten dokumentiert.

Ergebnis

In die Auswertung wurden 116 Intubationen eingeschlossen. Die Atemwegssicherung erfolgte zu 100 % mit ETI in ein, 2 oder 3 Intubationsversuchen (88,8, 10,3, 0,9 %); alternative Verfahren zur Atemwegssicherung mussten nicht eingesetzt werden. Eine Subgruppe mit immobilisierter Halswirbelsäule (HWS, n = 17) bot keine erhöhten Schwierigkeiten.

Schlussfolgerung

Das C-MAC®-Videolaryngoskop konnte durch die intubationserfahrenen Notärzte des beobachteten RTH bei der präklinischen Intubation von Traumapatienten sicher eingesetzt werden und verspricht eine hohe Erfolgsrate – dies bestätigt sich auch für eine Subgruppe von Patienten mit immobilisierter HWS. Verglichen mit den Ergebnissen anderer Studien zeigten sich ungünstige Laryngoskopiebefunde (C/L III + IV) reduziert.
