Primärer Megaureter

Zusammenfassung

Als Megaureter bezeichnet man einen prävesikal oder im Gesamtverlauf dilatierten Harnleiter, dessen Durchmesser mindestens 6 mm beträgt. Basisdiagnostik stellen die Sonographie und das Miktionszystourethrogramm dar. Zur Nierenfunktionsprüfung und Beurteilung der Harnabflussverhältnisse dient die Diureseszintigraphie. Die Unterscheidung zwischen einem primären, nicht-refluxiven, und einem primären, obstruktiv-refluxiven Megaureter ist für die therapeutische Entscheidungsfindung wesentlich. So ist der primäre, nicht-refluxive Megaureter in >80% der Fälle nicht operationsbedürftig, da die Spontanremissionsrate hoch ist. Dagegen ist der obstruktiv-refluxive Megaureter in der Regel korrekturbedürftig. Bei rezidivierenden Harnwegsinfekten/Pyelonephritiden sollte im 1. Lebensjahr eine Antibiotikadauerprophylaxe erfolgen.
