Prognostischer Wert der kardiovaskulären MRT bei Diabetikern

Zusammenfassung

Klinisches/methodisches Problem

Trotz eines im Durchschnitt erhöhten kardiovaskulären Risikos sind Diabetiker eine heterogene Population mit höchst unterschiedlichen individuellen Krankheitsverläufen, sodass eine fundierte Risikostratifizierung von Nöten ist.

Standardverfahren

Die klinische Untersuchung und die Bestimmung von Blutwerten dienen bei Diabetikern typischerweise zur Risikostratifizierung bzgl. kardio(zerebro)vaskulärer Ereignisse in der Zukunft.

Methodische Innovationen

Sowohl die Kardio- als auch die Ganzkörper-MRT stellen standardisierte Verfahren in der klinischen Diagnostik dar. Deren Stellenwert bei der Vorhersage von Ereignissen mit entsprechender Risikostratifizierung bei Diabetikern gewinnt basierend auf aktuellen Studienergebnissen zunehmend an Bedeutung.

Leistungsfähigkeit

Late Gadolinium Enhancement (LGE) im Kardio-MRT detektiert bei bis zu 30 % der Diabetiker stumme Myokardischämien, die mit einer Hazard Ratio von 3–6 mit kardiovaskulären Ereignissen assoziiert sind. Ebenfalls zeigen linksventrikuläre Wandbewegungsstörungen sowie eine erniedrigte Ejektionsfraktion einen prognostischen Wert. Im Ganzkörper-MRT sind der Vessel-Score und Karotisstenosen zusätzliche Prädiktoren für kardio(zerebro)vaskuläre Ereignisse.

Bewertung

Die MRT-basierte Vorhersage kardio(zerebro)vaskulärer Ereignisse bietet einen signifikanten Mehrwert zur klassischen Risikostratifizierung bei Diabetikern. Jedoch erlaubt nur die umfassende Ganzkörper-MRT-Untersuchung Patienten zu identifizieren, die über einen Zeitraum von 6 Jahren komplett frei von kardio(zerebro)vaskulären Ereignissen bleiben.

Empfehlung für die Praxis

Die Kardio-MRT, insbesondere die Erfassung von LGE, empfiehlt sich für die Vorhersage von Ereignissen. Die Ganzkörper-MRT stellt einen prognostischen Mehrwert dar, deren klinische Relevanz in weiteren Studien geklärt werden muss.
