Zusammenfassung

Hintergrund
Erkrankungen der Kopfhaut sind für die betroffenen Patienten nicht nur aufgrund des Juckreizes sehr belastend, sondern auch aufgrund der relativ guten Sichtbarkeit und der erschwerten therapeutischen Zugänglichkeit des behaarten Kopfes. Hier ist eine genaue Einordnung der Diagnose ein wichtiger Bestandteil zur adäquaten Therapiefindung.
Diagnostik
Wenn erythematöse und schuppige Kopfhautveränderungen das Leitsymptom darstellen, ist primär die Psoriasis von konstitutionellen Ekzemen (atopisch oder seborrhoisch) und von dem Kontaktekzem (allergisch oder toxisch-irritativ) zu unterscheiden. Die Untersuchung des restlichen Integuments ist zur Diagnosefindung unerlässlich. Auch die Anamnese gibt bedeutende Hinweise insbesondere bei dem Kontaktekzem.
Therapie
Therapeutisch kommen bei den genannten Kopfhauterkrankungen Lokalsteroide und hier besonders adhärenzfördernde Präparate wie Schaumformulierungen zur Anwendung. In Einzelfällen kann auch bei hohem Leidensdruck eine immunsuppressive Systemtherapie notwendig werden. Azolhaltige Antimykotika werden nicht nur beim seborrhoischen Ekzem, sondern auch bei der Psoriasis und bei der atopischen Dermatitis verwendet.
Zielsetzung
Der Beitrag gibt eine Übersicht über die klinischen Unterschiede und die therapeutischen Möglichkeiten bei der Psoriasis capitis sowie den unterschiedlichen Ekzemformen und beleuchtet die Differenzialdiagnose des irritativ-toxischen zum allergischen Kontaktekzem der Kopfhaut.
