Psychologische Diagnostik und Psychotherapie bei chronischen Schmerzen im Alter

Zusammenfassung

Bisher gibt es kaum systematische Übersichtsarbeiten zur psychologischen Diagnostik und Psychotherapie bei chronischen Schmerzsyndromen älterer Menschen. Dabei muss gerade bei älteren Menschen mit chronischen Schmerzen von einem deutlichen psychosozialen Handlungsbedarf ausgegangen werden. Der vorliegende Übersichtsbeitrag versucht, empfehlenswerte deutschsprachige Verfahren für die psychosoziale Diagnostik zu benennen sowie bisher vorliegende Studien zu psychologischen Therapieverfahren bei chronischen Schmerzen im Alter zusammenzufassen. Für die Diagnostik von kognitiven Fähigkeiten und schmerzspezifischen Einstellungen, Depression und Sturzangst, interpersonalen Prozessen und sozialen Aktivitäten, Schmerzbewältigung und Schmerzakzeptanz sowie Funktionsfähigkeit, psychologischem Wohlbefinden und Lebensqualität liegen größtenteils Verfahren mit guten psychometrischen Kennwerten vor. Erforderlich wären weitere Erfahrungen mit dem Einsatz der Instrumente bei kognitiv beeinträchtigten oder geriatrischen Patienten. Die Wirksamkeit altersangepasster kognitiv-behavioraler Verfahren und multimodaler Therapien ist belegt. Häufig mangelt es aber an Nachweisen hinsichtlich wichtiger Ergebnisparameter wie der Lebensqualität, Funktionsfähigkeit im Alltag oder auch Schmerzakzeptanz. Insgesamt sollte bei älteren Menschen von einem biopsychosozial-spirituellen Schmerzmodell ausgegangen werden. Religiosität bzw. Spiritualität sollte als möglicher Copingfaktor in der Forschung und täglichen Praxis mehr Beachtung erfahren. Achtsamkeits- und akzeptanzbasierte Therapieverfahren sollten auch in dieser Altersgruppe stärker untersucht werden.
