Rekonstruktion eines medialen patellofemoralen Ligaments mit Quadrizepssehnenstreifen

Zusammenfassung

Operationsziel

Stabilisierung der Patella durch Rekonstruktion des medialen patellofemoralen Ligaments.

Indikationen

Rezidivierende Patellaluxation nach lateral mit chronischer Schwächung des medialen patellofemoralen Ligaments (MPFL).

Kontraindikationen

Symptomatische femoropatellare Knorpelschäden vom Grad 3 oder höher nach ICRS (International Cartilage Repair Society). Tuberositas Tibiae Trochlear Groove Index (TTTG) >20 mm. Laterales Hyperkompressionssyndrom ohne patellare Instabilität.

Operationstechnik

3 cm lange quere Hautinzision über superomedialem Patellapol in 90°-Knieflexion. Längsinzision der Bursa praepatellaris und Darstellung der Quadrizepssehne. Präparation eines patellar gestielten Sehnenstreifens von 10 mm Breite, 3 mm Dicke und mindestens 8 cm Länge. Umschlagen und subperiostales Tunneln des Sehnenstreifens zum medialen Patellarand. Hindurchziehen des armierten Transplantats in der 2. Schicht der medialen Gelenkkapsel unterhalb des M. vastus medialis und der Faszie (1. Schicht) zur femoralen Insertion des MPFL [19]. Fixierung des Transplantats mittels bioresorbierbarer Interferenzschraube in einem Knochentunnel am femoralen Ansatz des MPFLs.

Weiterbehandlung

Patellazentrierende Orthese für 4 Wochen mit ROM („range of motion“) 0/0/90° und 20-kg-Teilbelastung für 3 Wochen. Ab der 4. Woche postoperativ Vollbelastung nach Maßgabe der Beschwerden. Beübung bis 90°-Flexion direkt postoperativ. Freigabe der Bewegung 6 Wochen postoperativ. Ergometertraining 6 Wochen postoperativ. Beginn mit Schwimmen und Lauftraining nach 10 Wochen. Rückkehr zu Risikosportarten 4–5 Monate postoperativ.

Ergebnisse

Zwischen März 2011 und November 2012 wurden 17 Patienten mittels der beschriebenen Technik versorgt. Davon waren 7 männlich und 10 weiblich mit einem Durchschnittsalter von 21,5±3,9 Jahren und einem durchschnittlichen BMI von 22,6±3,9 [4]. Es wurden nur Patienten mit mindestens einem Rezidiv der Patellaluxation nach konservativer Therapie eingeschlossen. Sehr hohe Patientenzufriedenheit nach 12 Monaten. Insgesamt 94,1% würden den Eingriff erneut durchführen lassen. Mit dem kosmetischen Ergebnis zufrieden waren 94,1%. Signifikante Verbesserung des Lysholm-Scores 6, 12 und 24 Monate postoperativ (p < 0,05). Freie Beweglichkeit 12 Monate postoperativ bei 94,1% der Patienten. Kujala-Score 12 Monate postoperativ 89±7,1 Punkte. Keine Rezidivluxationen 24 Monate postoperativ im untersuchten Kollektiv.
