Rolle der Bezugstherapeuten in der stationären Psychotherapie

Klinische Beobachtungsstudie zur Bedeutung für den symptombezogenen Behandlungserfolg

Zusammenfassung

Hintergrund

Während die Bedeutung des Therapeuten für das Behandlungsergebnis in der ambulanten Psychotherapie vielfach belegt ist, sind entsprechende Untersuchungen im stationären Setting rar.

Ziel der Arbeit

Die vorliegende Studie will bisherige Befunde zum Einfluss der Behandler auf das Therapieergebnis mithilfe einer naturalistischen Studie an stationären Psychotherapiepatienten erweitern.

Material und Methode

In einem naturalistischen Ansatz wurden differenzielle Effekte von 50 Bezugstherapeuten hinsichtlich der Symptombesserung der von ihnen im stationären Rahmen behandelten Patienten (n = 4758) analysiert. Die psychische Belastung wurde bei Aufnahme und Entlassung der Patienten mithilfe des Gesamt-Schwere-Index (GSI) der Symptom-Checkliste SCL-90-R gemessen. Wegen der geschachtelten Daten wurde eine Mehrebenenanalyse mit Zufallskoeffizienten durchgeführt.

Ergebnisse

Über die gesamte Patientenstichprobe reduzierte sich die Symptombelastung signifikant (d = 0,75). Der Einfluss der einzelnen Therapeuten auf das symptombezogene Behandlungsergebnis war gering und klärte lediglich 0,5 % der Varianz auf. Die Varianzaufklärung stieg jedoch mit zunehmender Symptombelastung der Patienten bei Aufnahme.

Schlussfolgerung

Die Befunde deuten darauf hin, dass der Einzeltherapeut in der stationären Psychotherapie weniger bedeutsam ist als im ambulanten Setting. Dies ist auf den multipersonalen Bezugsrahmen im Setting der Krankenhausbehandlung zurückzuführen.

Ein Random-intercept-Modell (ohne „random slope“) führte bezüglich der Therapeutenvarianz zu einem sehr ähnlichen Ergebnis (0,36 %); deshalb wird dieses Modell nicht im Detail dargestellt.

Die Analysen wurden nach Ausschluss von 661 Patienten, die zu Beginn GSI-Werte > 0,7 aufwiesen (d. h., bei denen es sich möglicherweise um sog. Pseudogesunde handelt; Spitzer et al. 2013) wiederholt. Die Ergebnisse zur Therapeutenvarianz waren praktisch identisch (0,43 %); deshalb werden diese Analysen nicht im Detail dargestellt.
