Schmerzerfassung in der Routineversorgung

Eine prospektive Beobachtungsstudie auf einer orthopädischen Station

Zusammenfassung

Hintergrund

Obwohl gut etablierte Leitlinien Empfehlungen zur Anwendung von Analgetika, zur Schmerzmessung und zur Organisation in der Schmerztherapie geben, leiden immer noch viele Patienten unter z. T. vermeidbaren starken Schmerzen. Wir vermuteten, dass einer der Gründe die nach wie vor unzureichende Thematisierung von Schmerzen im alltäglichen Patientengespräch ist. Mit unserer Studie verfolgten wir das Ziel, das Ausmaß dieser Schmerzthematisierung in der orthopädischen Routine zu evaluieren.

Material und Methoden

In einer prospektiven Beobachtungsstudie auf einer orthopädischen Station eines Universitätsklinikums waren Ärzte bei der morgendlichen Visite und Pflegedienstmitarbeiter bei ihren abendlichen Pflegerunden eingeladen, sich von unabhängigen, geschulten Beobachtern begleiten zu lassen. Die Beobachter erfassten systematisch alle für die Schmerzeinschätzung relevanten Aspekte, die im Anschluss deskriptiv analysiert wurden.

Ergebnisse

Die Beobachter dokumentierten 572 Patientengespräche von 7 Ärzten mit 108 Patienten und 578 Gespräche von 12 Pflegedienstmitarbeitern mit 102 Patienten. Ärzte und Pflegedienstmitarbeiter fragten in 20 bzw. 16 % der Gespräche nach Schmerzen. Während die Patienten sich in den ärztlichen Visiten am häufigsten zu ihren aktuellen Schmerzen äußerten (in 35 % der Gespräche), thematisierten sie gegenüber der Pflege am häufigsten ihren Analgetikabedarf (in 52 %). Zur Schmerzintensität äußerten sich die Patienten in 16 (Arztvisite) bzw. 17 % (Pflegerunde) der Gespräche.

Schlussfolgerung

Mittels eines umfassenden Monitoring-Konzepts haben wir gezeigt, dass eine systematische Schmerzerfassung als Basis für eine systematische, individualisierte Schmerztherapie in unserem Setting weder bei Ärzten noch bei Pflegedienstmitarbeitern optimal in der Routineversorgung implementiert war.
