Seltene renale Ablagerungserkrankungen

Fibrilläre, immunotaktoide und kryoglobulinämische Glomerulonephritis

Zusammenfassung

Hintergrund

Seltene renale Ablagerungserkrankungen wie die fibrilläre Glomerulonephritis (GN), die immunotaktoide GN und die kryoglobulinämische GN umfassen nur etwa 1% aller Eigennierenerkrankungen, sind in ihrer Ätiologie und Pathogenese nicht hinreichend verstanden und oft schwierig zu diagnostizieren. Im Folgenden sollen einige klinische und morphologische Charakteristika beschrieben werden: Die fibrilläre GN, die nur sehr selten eine Assoziation mit B-Zell-Neoplasien wie bei den anderen Erkrankungen mit organisierten Ablagerungen zeigt, befällt ausschließlich die Nieren und präsentiert sich klinisch typischerweise mit einer großen, meist nephrotischen Proteinurie und einer unterschiedlich ausgeprägten Hämaturie. Etwa 50% der Patienten entwickeln eine terminale Niereninsuffizienz nach 2 Jahren. Eine reproduzierbar effektive Therapie ist bislang unbekannt. Eine Rekurrenz der Erkrankung im Nierentransplantat ist bereits nach 2 Jahren beschrieben, es sind aber auch Transplantationen mit einer späteren Rekurrenz (7–9 Jahre) bzw. ohne Rekurrenz dokumentiert. Die Lichtmikroskopie kann sehr variabel sein, die Kongorotfärbung ist im Gegensatz zur Amyloidose negativ, und in der Immunhistologie finden sich vor allem mesangiale und membranöse IgG- und C3c-Ablagerungen. Die definitive Diagnose der fibrillären GN wird jedoch erst elektronenmikroskopisch erbracht, wo sich charakteristische fibrilläre Ablagerungen mit einer Größe von 12–24 nm finden. Die immunotaktoide GN ist noch deutlich seltener als die fibrilläre GN, wobei die beiden Erkrankungen ähnliche klinische, lichtmikroskopische und immunhistologische Befunde zeigen und lediglich durch elektronenmikroskopische (EM-)Untersuchungen voneinander abgegrenzt werden können. Hier finden sich bei der immunotaktoiden GN keine Fibrillen, sondern charakteristische Mikrotubuli mit einer Größe von meist mehr als 30 nm. Die kryoglobulinämische GN als renale Beteiligung findet sich bei etwa 10–40% der Patienten mit Kryoglobulinämie, meist in Form einer diffus proliferativen GN mit charakteristischen intraglomerulären hyalinen Thromben, die in der Immunhistologie positiv für IgG und/oder IgM sind. Elektronenmikroskopisch finden sich dann häufig strukturierte Depositen in Form von fibrillären, tubulären oder tubuloretikulären Ablagerungen, die eine Ähnlichkeit zur fibrillären und immunotaktoiden GN erkennen lassen.

Schlussfolgerung

Die seltenen renalen Ablagerungserkrankungen umfassen eine heterogene Gruppe von Krankheiten, die neben der in der Niere recht häufigen Amyloidose v. a. auch seltenere und diagnostisch schwierige Nierenerkrankungen wie die fibrilläre GN, die immunotaktoide GN und die kryoglobulinämische GN umfassen, zu deren definitiver Einordnung die ultrastrukturelle Untesuchung mittels Elektronenmikroskopie unabdingbar notwendig ist.
