Suprakondyläre Humerusfrakturen des Erwachsenen

Hintergrund

Insbesondere unter dem Aspekt der demografischen Entwicklung kann von einer deutlichen Zunahme suprakondylärer Humerusfrakturen ausgegangen werden.

Diagnostik

Aufgrund der anatomischen und biomechanischen Gegebenheiten am distalen Humerus bedarf jede Fraktur einer exakten klinischen und radiologischen Untersuchung, um die optimale Versorgung zu ermitteln. Im Rahmen der klinischen Untersuchung sollte insbesondere der neurovaskuläre Status exakt erhoben werden.

Therapie

Ziel der operativen Versorgung ist eine möglichst exakte und stabile Rekonstruktion der anatomischen Gegebenheiten, um eine frühfunktionelle Nachbehandlung zu ermöglichen und somit einer langfristigen Funktionseinschränkung entgegenwirken zu können. Die gängigsten Versorgungsmethoden sind die Doppelplattenosteosynthese (Kleinfragmentrekonstruktionsplatten/winkelstabile Plattensysteme) und zunehmend die Implantation einer Totalendoprothese bei sehr distalen Frakturformen und Osteoporose. Eine intraoperative Darstellung und der Schutz des N. ulnaris sind obligat. Ein konservatives Vorgehen bei suprakondylären Humerusfrakturen ist in der heutigen Zeit nur noch selten bei einfachen Frakturformen beim älteren Patienten oder beim Vorliegen absoluter Kontraindikationen gegen eine operative Therapie angezeigt. Die gefürchtetsten Komplikationen distaler Humerusfrakturen sind Läsionen des N. ulnaris, heterotope Ossifikationen und die verzögerte oder ausbleibende Frakturheilung.
