Zusammenfassung

Hintergrund

Das Erreichen einer lokalen Tumorkontrolle bietet für viele Patienten im Stadium I des nichtkleinzelligen Lungenkarzinoms (NSCLC) eine definitive Heilungschance.

Methode

Recherche und Auswertung aktueller Literatur sowie klinischer Studien.

Ergebnisse

Mit Einführung risikoadaptierter Screeningverfahren wird die Zahl von Patienten in günstigen Stadien wahrscheinlich steigen. Höheres Lebensalter und eine Vielzahl oft bestehender Komorbiditäten und Einschränkungen stellen dabei besondere Anforderungen an das therapeutische Vorgehen. Mit Operation und stereotaktischer ablativer Radiotherapie (SART) stehen zwei lokal ähnlich effektive Verfahren für die Behandlung des frühen Lungenkarzinoms zur Verfügung. Der kurative Effekt der onkologisch radikalen Tumorresektion ist über Jahre erwiesen. In eindeutig operablen Fällen liegen die Vorteile der sicheren Diagnose, radikalen Entfernung und Definition von Risikokollektiven für adjuvante Maßnahmen deshalb auf der Hand. Die SART ist weniger invasiv und erweitert darüber hinaus das Behandlungsspektrum auch auf Patienten, die vorher nur supportiv behandelt werden konnten.

Schlussfolgerung

Speziell unter Berücksichtigung von Alter, Komorbiditäten und Patientenpräferenz müssen die interdisziplinäre Abwägung und Beratung des Patienten über das für ihn beste Vorgehen heute Standard sein.
