Therapie der Aortenklappenstenose

Zusammenfassung

Die kalzifizierende Aortenklappenstenose ist der häufigste Herzklappenfehler in der westlichen Welt. Sie ist eine degenerative und chronisch-progressive Erkrankung insbesondere des höheren Alters. Aufgrund der demografischen Entwicklung nimmt sie stetig zu. Es besteht keine medikamentöse Behandlungsoption. Die einzige Therapiemöglichkeit bei hochgradiger Aortenklappenstenose ist somit der Klappenersatz. Die Echokardiographie ist das diagnostische Mittel zur Bestimmung des Schweregrads der Stenose sowie zur Untersuchung der Aortenklappenmorphologie. Als hochgradig gilt die Aortenklappenstenose bei einer Klappenöffnungsfläche < 1,0 cm2, einem Klappenöffnungsflächenindex < 0,6 cm2/m2 Körperoberfläche, einem mittleren Druckgradienten > 40 mmHg sowie einer maximalen Flussgeschwindigkeit > 4,0 m/s. Die Entität der Aortenklappenstenose mit niedrigem Fluss und niedrigem Gradienten ist komplex, ihre Diagnostik und Behandlung stellt immer noch eine Herausforderung dar. Asymptomatische Patienten haben eine gute Prognose, müssen aber regelmäßig in Bezug auf Symptome sowie Hinweise einer Progression reevaluiert werden. Treten die Kardinalsymptome Luftnot und Leistungsminderung, Angina pectoris oder Synkope auf, verschlechtert sich die Prognose dramatisch und ein Aortenklappenersatz ist indiziert. Der therapeutische Goldstandard ist der chirurgische Ersatz. Für Hochrisikopatienten – im hohen Alter und/oder mit bedeutender Komorbidität – hat sich der Transkatheteraortenklappenersatz als Standardverfahren etabliert.
