Management der Komplikationen in der Thoraxchirurgie

Zusammenfassung

Hintergrund

Komplikationen sind nicht immer vermeidbar und ihre Behandlung ist integraler Bestandteil einer hochwertigen medizinischen Versorgung. Komplikationen an großen Atemwegen sind selten, erfordern aber unbedingt die Mitbehandlung durch einen versierten Thoraxchirurgen.

Ziel der Arbeit

Der Leser soll Maßnahmen zur Komplikationsvermeidung, Früherkennung und zur Komplikationsbehandlung kennenlernen, um die Notwendigkeit für eine frühe interdisziplinäre Zusammenarbeit zu verstehen.

Material und Methoden

Eine selektive Literaturrecherche wurde durch eigene Erfahrungen ergänzt und mit prospektiv gewonnenem Bildmaterial ausgestattet.

Ergebnisse

Für das Auftreten aller genannten Komplikationen gibt es Risikokonstellationen, die der Chirurg erkennen sollte, um gezielt Maßnahmen der Früherkennung von Komplikationen zu ergreifen. Trachealverletzungen und Bronchusstumpfinsuffizienzen sind selten (< 5 %), dagegen kann eine Rekurrensparese nach linksseitiger Pneumonektomie in bis zu 30 % der Fälle auftreten.

Diskussion

Spätestens nach Auftreten einer Komplikation ist es wichtig, thoraxchirurgisch umgehend und konsequent zu handeln und, wenn erforderlich, andere Fachdisziplinen einzuschalten, um weiteren Schaden vom Patienten abzuwenden.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
