Zusammenfassung

Die „video-assisted thoracic (thoracoscopic) surgery“ (VATS) hat weltweit die Tätigkeit der Thoraxchirurgen revolutioniert und die Behandlung der Patienten verbessert. In den letzten Jahrzehnten hat sich die VATS enorm weiterentwickelt und macht heute in vielen thoraxchirurgischen Kliniken ca. 50 % der operativen Eingriffe aus. Die VATS ist weniger invasiv und ermöglicht so eine schnellere Erholung des Patienten. In den letzten 20 Jahren hat sich die VATS zu einer sicheren und effektiven Technologie in der Diagnostik und Therapie vieler thorakaler Erkrankungen entwickelt. Mit zunehmender Erfahrung konnten auch komplexere und technisch anspruchsvollere Eingriffe durchgeführt werden. Heute ist die VATS eine in vielen Fällen der Thorakotomie überlegene Technik.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
