Paradigmenwechsel in der Ernährungstherapie

Zusammenfassung

Herzinsuffizienz bleibt mit ihrer schlechten Prognose ein zentrales Problem der westlichen Welt. Die zunehmende Prävalenz von diastolischem Herzversagen (HFPEF, heart failure preserved ejection fraction) beruht vielfach auf myokardialem Energiemangel durch die metabolischen Auswirkungen von Insulinresistenz. Dies lässt angesichts der Inzidenz von metabolischem Syndrom / Typ-2-Diabetes ein epidemisches Ausmaß befürchten; und dabei fehlen evidenzbasierte Therapieempfehlungen noch immer. Der Bekanntheitsgrad der hierfür relevanten Interaktionen im Myokard wächst wegen ihres interdisziplinären Charakters (Endokrinologie und Kardiologie) nur langsam mit weiterhin unzureichender Ausschöpfung des gesundheitsförderlichen Potenzials im Wissen um das Phänomen Insulinresistenz. Dabei liegt der Grundstein dieser metabolischen Erkrankung in der diätetischen Normalisierung. Diese Übersichtsarbeit stellt deshalb im ersten Teil anlässlich einer Pilotstudie die diätetische Verbesserung der diastolischen myokardialen Dysfunktion und aller Manifestationen des metabolischen Syndroms vor und im zweiten Teil die relevante Pathophysiologie und rationale und evidenzbasierte Therapie des Stoffwechsels bei Diabetes / metabolischem Syndrom und damit insgesamt einen Paradigmenwechsel in der Ernährungstherapie.
