Zusammenfassung

Hintergrund

Vertikale Defizite des knöchernen Kieferkamms stellen die technisch anspruchsvollste Defektkonfiguration dar. Zum Ausgleich dieser Defizite gibt es mehrere Möglichkeiten in der Implantologie. Hierzu gehören die gesteuerte Knochenregeneration (Guided Bone Regeneration, GBR), Onlay- oder Inlaytechniken, Distraktionsosteogenesen und Kombinationen aus diesen Verfahren. Sie werden in Abhängigkeit vom Defekttyp eingesetzt.

Methoden

Die Distraktionsosteogenese bietet den Vorteil, dass kein Knochen oder Knochenersatzmaterial eingebracht werden muss und zusätzlich Weichgewebe gewonnen werden kann. Grundsätzlich ist dazu eine gewisse Restknochenhöhe erforderlich. Die Inlaytechnik oder Sandwichosteoplastik bietet aufgrund der originären Vaskularisation der umgebenden Gewebe ein sehr gutes Lager für den einzubringenden Knochen oder das Knochenersatzmaterial, sodass dieses schnell erschlossen werden kann. Neben einem erforderlichen Mindestrestknochenangebot ist der maximale vertikale Knochengewinn durch die Dehnbarkeit des lingualen bzw. palatinalen Weichgewebes limitiert. Bei ausgedehnten Defekten wird bei der Onlaytechnik und der GBR i.d.R. autologer Knochen verwendet, was eine additive Entnahmemorbidität zur Folge hat. Mit allen Methoden lassen sich in den Händen des Geübten gute Ergebnisse erzielen, sodass hier auch die individuelle Erfahrung eine Rolle bei der Auswahl spielt. Alternativ bieten kurze Implantate die Möglichkeit, die Indikation und das Ausmaß vertikaler Augmentationen zu reduzieren. Daher sollte der Besprechung von Behandlungsalternativen bei der Aufklärung des Patienten eine besondere Rolle zukommen.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
