Die Wirksamkeit von Cumarinen und direkten oralen Antikoagulanzien (DOAK) hinsichtlich der Schlaganfallprävention bei Patienten mit Vorhofflimmern/-flattern wurde in zahlreichen Studien nachgewiesen. Das Schlaganfallrisiko wird je nach Patientenkollektiv um 50– 70% gesenkt [1, 2, 3, 4, 5, 6]. Es ist jedoch ebenfalls erwiesen, dass die überwiegende Mehrheit der Patienten das jeweilige Antikoagulans nicht auf Dauer einnehmen kann [7, 8]. Die häufigsten Gründe hierfür sind Unverträglichkeiten, Blutungskomplikationen, Incompliance und Zurückhaltung des behandelnden Arztes aufgrund befürchteter Nebenwirkungen. Die Realität zeigt, dass (und zwar unabhängig vom Alter) nach zwei Jahren nur noch ca. 60% der Patienten antikoaguliert sind, nach fünf Jahren nur noch 40% und nach sechs Jahren nur noch ca. 30% der Patienten [7, 8]. Auch für die neueren Antikoagulanzien (Dabigatran, Apixaban und Rivaroxaban) zeigt sich, selbst unter kontrollierten Studienbedingungen, eine mit Marcumar identische Absetzrate [4, 5, 6]. Die 50–70%ige Risikoreduktion bzgl. zerebraler Ischämien durch orale Antikoagulanzien gilt somit nur für den über die Zeit stetig kleiner werdenden Anteil an Patienten, die diese Substanzen auch wirklich einnehmen. …
