Zerebrale Amyloidangiopathie und Demenz

Zusammenfassung

Die zerebrale Amyloidangiopathie (CAA) ist eine der häufigsten Ursachen intrazerebraler Blutungen. Zugrunde liegt eine degenerative Gefäßerkrankung, die durch Ablagerungen von Betaamyloid in den Wandschichten zustande kommt. Die in der Kongorotfärbung nachweisbaren Amyloiddeposits sind biochemisch dem Material in den senilen Plaques der Alzheimer-Demenz ähnlich. Von den Amyloidablagerungen sind vor allem die kortikalen Gefäße betroffen mit Formation von Mikroaneurysmen und fibrinoider Wandnekrose. Die erhöhte vaskuläre Vulnerabilität erklärt die häufig simultan und multipel auftretenden intrazerebralen Blutungen bei der CAA. Daneben besteht eine Korrelation zwischen zerebraler Amyloidlast, Marklagerdystrophie, Mikroblutungen und der Entwicklung einer Demenz. Die Magnetresonanztomographie (MRT) mit Gradientenechosequenzen ist die Methode der Wahl zum Nachweis auch kleinster Mikroblutungen und umschriebener kortikaler Siderosen, welche zu flüchtigen Symptomen führen können. An eine entzündliche CAA-Variante ist vor allem dann zu denken, wenn progrediente kognitive Defizite sich mit Kopfschmerzen, epileptischen Anfällen und multifokalen Symptomen kombinieren. Histopathologisch werden die CAA-“related-inflammation“ (CAA-ri) mit perivaskulären entzündlichen Infiltraten und die „amyloid-β-related-angiitis“ (ABRA) mit histologischem Nachweis einer transmuralen Vaskulitis unterschieden. Bei den entzündlichen Varianten sollte eine Behandlung mit Kortikoiden und Immunsuppressiva erfolgen.
