Zirkumzision des Mannes aus infektiologischer Sicht

Zusammenfassung

Hintergrund

Im Mai 2012 stellte das Landgericht Köln fest, dass die Beschneidung von Minderjährigen aus religiösen Gründen eine rechtswidrige Körperverletzung sei. Dies führte zu erheblichen politischen und religiösen Diskussionen, da Juden (Brit Mitla) und Muslime (hitân) die Beschneidung als unverzichtbaren Bestandteil ihrer Religion betrachten. Noch im gleichen Jahr wurde vom Bundesrat das „Beschneidungsgesetz“ verabschiedet, das die Beschneidung von Jungen „nach den Regeln der ärztlichen Kunst“ weiter erlaubt.

Fragestellung

Wie ist die Zirkumzision aus infektiologischer Sicht zu beurteilen (pro und kontra)?

Methode

Es erfolgte eine Literaturrecherche zu den Risiken der operativen Entfernung der Vorhaut; zu Infektionen durch den Eingriff sowie zu Langzeitauswirkungen auf das Übertragungsrisiko sexueller Infektionen (HIV, Herpes, HPV, Syphilis, Chlamydien-Infektionen, Ulcus molle und Gonorrhö).

Ergebnisse

Kontra Zirkumzision: Im Säuglingsalter ist, abhängig von der Art des Eingriffs, mit bis zu 10 % Wundinfektionen, 8–20 % Entzündungen des Meatus urethrae (eventuell Meatusstenosen) und 2 % Harnwegsinfektionen zu rechnen. Blutungskomplikationen sind häufig (35 %), während Penisnekrosen und letale Sepsis sehr selten (geschätzte Häufigkeit 1:20.000) auftreten. Pro Zirkumzision: Beschnittene Kinder haben später ein deutlich geringeres Risiko für Harnwegsinfektionen (1:10) und beschnittene Männer ein um etwa 50–60 % verringertes Risiko, sich mit viralen sexuell übertragbaren Infektionen (STI) wie HIV, HPV und HSV zu infizieren. Für die Übertragung von Treponema pallidum, Haemophilus ducrey und anderen bakteriellen Erregern ist dieser Vorteil einer Zirkumzision nicht ausreichend gesichert.

Diskussion und Schlussfolgerungen

Die Zirkumzision im Säuglings- und Kindesalter ist ein irreparabler Eingriff in die körperliche Unversehrtheit, für den es selten eine medizinische Indikation gibt. Das Komplikationsrisiko ist abhängig von der Ausbildung des Beschneiders, Narkose- und Hygieneverhältnissen. Zirkumzisionen sollten nur unter optimalen chirurgischen und hygienischen Verhältnissen bei aufgeklärten und selbstbestimmten jungen Männern durchgeführt werden. Im Jugendlichen- und Erwachsenenalter reduzieren Zirkumzisionen das Übertragungsrisiko vor allem viraler STI (HIV, HSV, HPV), eventuell auch von Treponema pallidum und Haemophilus ducrey (unzureichende, widersprüchliche Studiendaten). Da jedoch weitere Faktoren wie sexuelles Risikoverhalten (ungeschützter Verkehr, Promiskuität) und die Durchseuchungsrate der Bevölkerung die STI-Übertragung beeinflussen, ist die Rolle der Zirkumzision als effektive Maßnahme zur Vermeidung von STI-Infektionen umstritten.
