Posterior reversible encephalopathy syndrome is described as acute neurological deterioration characterized by headache, visual changes and seizures. It is characterized by radiologic findings of vasogenic edema in the parieto-occipital lobes. Methotrexate-induced toxicity can cause PRES, most of the cases being in the pediatric age group and is rare in adults. Also, it is important to note that even in adults most of the cases described are after intrathecal administration of methotrexate. We report a case of posterior reversible encephalopathy syndrome (PRES) in a patient previously on low dose methotrexate (MTX) for rheumatoid arthritis found to have methylenetetrahydrofolatereductase (MTFHR) gene mutation-c677t homozygous. …
